Replacement Card and Other Forms

Instant Certification Card Replacement 

 

The following procedure is to be used when one or more of the following conditions apply:

· Your PADI Diver certification was lost.

· Your PADI Diver certification was damaged.

· You changed you name (include a copy of legal documentation of name change.)

· You are upgrading from any PADI Junior Diver certification.

· You would like to replace your existing card with a new Project AWARE Edition Certification Card.

PADI’s new Certification Online and Dive Chek systems allows us to issue you a temporary certification card here at the store instantly. All you’ll need to do is come in the store so we can take your picture with our digital camera. We’ll hand you a temporary card with your photo on it. Then in a week or two you’ll get your new card in the mail.

PADI Medical Release Form

The following form is to be take to your physician so that he or she may determine if your medical condition prohibits you from participating in scuba diving.  Please follow the instructions on the form.
